This form is for those who want to ADOPT a rat from the Rockstar Rats Retreat

This form will also be used as your adoption contract. You will be asked to sign a copy of this form during the time of adoption.

*Previous rat experience (first timers are great too):
*What type of cage and bedding will be used? Store bought bedding must be placed in a freezer for at least a week before use to ensure that any parasites such as mites, or eggs will be dead before use. Will you do this?
*What will their diet consist of? Store bought food must be placed in a freezer for at least a week before use to ensure that any parasites such as mites, or eggs will be dead before use. Will you do this?
*Do you have any other rats? If so how what are their genders and how many? Are there other pets at home?
*Please read each statement carefully. All boxes must be checked to adopt from Rockstar Rats Retreat:
The animal(s) being adopted is for myself as a companion and family member.
The animal(s) being adopted will not be used as a breeder by me or anyone else.
The animal(s) being adopted will not be used as a feeder, and will not be sold or given away as a feeder.
I agree that the animal(s) will be treated humanely, and will be provided with fresh food, water and a clean living environment with adequate space.
I will obey all applicable laws governing control and custody of animals.
I understand that unless I have proof of a same sex or spayed/neutered rat buddy/buddies waiting at home, I must adopt a same sex pair.
I understand that if I am under 18, a parent or guardian will have to accompany and sign for me at the time of adoption. If the animal(s) are for a child, proper supervision will be given at all times.
I have or will acquire the information of a vet who cares for rodents, and I will provide proper medical care and treatment for my animal(s) should the need arise.
The animal(s) will be properly quarantined for 14 days if there are other rats in the house. It is to the best of Rockstar Rats knowledge that the animal(s) are healthy and free from any illness or contagious disease. If any illness occurs within the quarantine period, I understand that I can return the adopted animal(s) to Rockstar Rats and receive a refund. I understand that if any illness or medical problems occur after the quarantine period, Rockstar Rats is not liable for any medical bills or refunds.
I understand that Rockstar Rats Retreat assumes no responsibility, nor shall they be held liable for any damages caused by the adopted animal(s) to property, person, or other pets.
Rockstar Rats Retreat will be informed of my current contact information and is given permission to contact me at any reasonable time to ensure the adopted animal(s) are being cared for properly.
The adopted animal(s) will not be sold, given away, or abandon for any reason. If it becomes necessary to find the animal(s) another home, I will contact Rockstar Rats Retreat and return the animal without requesting a fee, or to find a re-homing solution together.
